Designing proper paperwork that is easy for a patient to fill out AND for you to use is NEVER discussed.  We will review what basic paperwork is needed or required and review what other options you have available that match your practice values & scope.

an EXAM to do.
Not all patients require the same type of exam however ALL patients require a basic NMS exam!  Many doctors mistake a chiropractic exam for an NMS exam... I will show you how to do a routine exam including basic functional movements in 7 minutes!
